Mom of four wins $14M jackpot after accidentally buying lotto ticket

    By Samantha Tata, NBCLosAngeles.com

Accidentally spending $1 more than usual on Lotto Tickets paid off for a mother of four sons in Orange County to the tune of $14 million.

Thuan Le bought her winning SuperLotto Plus last week at a Mission Viejo, Calif., CVS (map), where she typically buys $4 worth of Powerball tickets and $1 on a SuperLotto Plus ticket.

When Le noticed she accidentally put $6 into the self-service machine, she took her chances on an extra – would-be winning – SuperLotto Plus ticket.

Le checked the winning ticket with her friend. After ticking off the matching numbers one by one, Le was so excited, she ran screaming from the store, according to a news release from California Lottery.

A single mother of four sons, Le said she plans to use her winnings to buy a house, travel and visit her parents in Vietnam.

    More news from NBCLosAngeles.com

One of Le’s sons thought his mother was kidding when she called, exuberant to share the news.

"We thought she was joking," her son told Lotto officials, "but we thought would she really joke like this? My older brother said, 'yes, she would.'"

Le’s lucky numbers were 5, 33, 25, 46, 32, and the Mega number 26. CVS Pharmacy will receive a bonus of one half percent, or $70,000, for selling the winning ticket.

An undetermined roadside ignition in grass and debris off the Ventura Freeway in Southern California sparked the Springs Fire, officials said on Sunday.

Officials are calling it an "undetermined roadside ignition of grass and debris" that started the region's largest wildfire so far this year that scorched nearly 44 acres along the Ventura and Los Angeles county lines.

Now that firefighters have the Springs Fire nearly under control after three days fighting in steep, rugged and rocky terrain, the focus of the blaze turns to a grassy area off the Ventura Freeway in the Conejo Grade where the fire was sparked on Thursday.

Full coverage from NBCLA: Southern California Wildfires

The fire began off the southbound side of the Ventura Freeway at Camarillo Springs Road. The fire quickly spread to over 28,000 acres of the Santa Monica Mountains in near triple-digit heat and low humidity.

Some 15 homes and several recreational vehicles were damaged and several communities in the path were evacuated. No injuries were reported and no one died.

The fire prompted a massive mutual-aid response from agencies across the state.

At one point, 9,000 firefighters — assisted by water-dropping and fire-retardant dropping planes and helicopters — attacked the flames.

The fire was 60 percent contained on Sunday, officials said.

Man charged with murder accidentally released and sheriff wants him back

    L.A. County Sherrif's Dept. / AP

This photo released Friday by the Los Angeles County sheriff shows a Johnny Mata, who is charged with murder and was accidentally released by authorities from jail last month because of a clerical error.

    By Brandon Lowrey, NBCLosAngeles.com

A clerical error caused Los Angeles County sheriff's deputies to release a jail inmate charged with gang-related murder last month, and authorities have asked the public to help them find him.

    Follow @NBCNewsUS

Johnny Mata, 32, was released in downtown Los Angeles on April 4 after he was charged with a 2010 gang murder in Baldwin Heights, according to a Los Angeles County Sheriff's Department statement.

    See original report at NBCLosAngeles.com

"Mata’s release should not have occurred because a murder charge had been filed by the District Attorney and a processing clerk had not yet entered it as a 'hold' on inmate Mata," the statement said.

Mata was arrested May 25, 2012 by the El Monte Police Department. His last court date was listed as April 3 -- the day before his accidental release.

Mata was described as a Latino man, 6 feet, 1 inch tall, 197 pounds.

Anyone with information about Mata's whereabouts can call sheriff’s homicide detectives at (323) 890-5500. Anonymous tipsters call LA Crime Stoppers at (800) 222-TIPS (8477), texting the letters TIPLA plus your tip to CRIMES (274637), or going to lacrimestoppers.org

Residents evacuate as 3,000-acre wildfire threatens Southern California homes

As the Santa Ana winds kick up across Southern California the gusts are fueling a blaze burning west of Palm Springs that is damaging homes and threatening many more. NBC's Miguel Almaguer reports.

By Christina Cocca and Tony Shin, NBCLosAngeles.com

A wind-driven wildfire that threatened homes near Banning in Riverside County that forced evacuations and destroyed at least one home continued to grow Wednesday evening, scorching nearly 3,000 acres.

    Follow @NBCNewsUS

At least 425 firefighters, 46 engine companies, 16 fire crews, six helicopters and six air tankers were sent to control the 3,000-acre two-headed blaze, which sent up thick towers of smoke.

The massive blaze, dubbed the Summit Fire, was at 30 percent containment as of 7:30 p.m. The fire was 35 percent contained by 9 p.m. and had a "slow rate of spread," according to Riverside County Fire Department's online incident report.

The blaze was reported at 12:38 p.m. at 10 acres. By 6 p.m., it had grown to consume at least 1,700 acres.

At least one home burned on Mesa Street at Sunset Avenue (map), and about 700 people had been evacuated and moved to aid centers. All evacuations were lifted by 8 p.m., according to Riverside County Fire officials.

    View more videos at: http://nbclosangeles.com.

At least one firefighter suffered minor injuries while battling the blaze. Fire Chief John Hawkins of CAL Fire's Riverside Unit said the first responder suffered burns to the face.

Regional air quality regulators issued a smoke advisory for the area around Banning, meaning people should avoid unnecessary outdoor activities anywhere smoke can be seen or smelled.

Hawkins said crews from outside Riverside County were called to battle the blaze in the rolling foothills and canyons.

"We are bringing in everything we can to get a hold on this fire," he said, adding that the area had very difficult access.

Hawkins said there are two "heads" to the fire: one north of Banning that is heading west toward an area north of the neighboring city of Beaumont; and another in the rugged foothills headed in the direction of the apple-growing community of Oak Glen.

Winds began to slow down about 6 p.m.

"We're looking a lot better," Hawkins said at the time, adding that the blaze is still "nowhere near contained."

The Banning Police Department evacuated residents on Indian School Lane near Eighth Street. Roads were closed on Wilson Street between San Gorgonio Road and Highland Springs Avenue.

More than 200 families in the Highland Springs Mobile Home Park were also evacuated. Those orders were lifted at 8 p.m.

Banning PD asked residents in the Bench area just north of Banning to remain in their homes because surrounding roads were blocked by fire.

Evacuated residents were ordered to go to the Banning Community Center at 789 N. San Gorgonia Avenue.

An evacuation center for small animals has been set up at San Jacinto Valley Animal Services located at 581 S. Grand Ave. in San Jacinto. A large-animal evacuation center is located at Noble Creek Park, 390 Oak Valley Parkway in Beaumont.

Crews from Riverside County Fire Department and CAL Fire worked with multiple agencies  -- including from as far away as Orange County -- to control the wildfire that started near North San Gorgonio Avenue and Summit Drive in Banning (map).

"We had an abnormally early start (to the fire season)," Hawkins said. "This is very concerning."

Areas of Southern California on Wednesday were under a red flag warning, or increased wildfire danger, which indicates a dangerous combination of high winds, hot temperatures and low humidity.

Worker chips away with hammer after being pinned by 1,000-pound slab of granite

    View more videos at: http://nbclosangeles.com.

By Samantha Tata, Alex Calder and Beverly White, NBCLosAngeles.com

Pinned between a massive slab of granite and the wall of a cargo container, a worker who appeared to aid in his own extrication was rescued Tuesday afternoon in Fontana, Calif.

The 1,000-pound piece of rock, often used for countertops, was being delivered to a contracting company in Fontana. The trapped worker apparently was helping take the slab out of the truck when it split in two.

Crews from the San Bernardino County Fire Department cut open part of the truck's rear wall, allowing the worker to dangle his right arm outside the truck.

"As soon as they cut a section out, his arm fell out," witness Lucky Lira said. "At that point they were frantically trying to get him out."

The worker appeared to be helping rescuers break down the granite that was trapping him, at one point taking a hammer from crews to chip away at the rock.

    More from NBCLosAngeles.com

"He got quite lucky," said Battalion Chief Kathleen Opliger, who led the rescue. "When the granite came down, it formed an alcove for him, putting pressure on his lower extremities instead of his whole body."

Paramedics hooked up the man to an IV drip attached to what appeared to be a saline bag taped to the outside of the truck.

The man became trapped in the truck about 5 p.m. (8 p.m. ET) on Industry Avenue.

Rescue crews pulled the man out of the truck about 6:30 p.m. He appeared to be moving his arms freely and speaking with rescuers as he was airlifted to the hospital.

Witnesses called the rescue a "miracle."

"He grabbed hold of his rescuer, was trying to hug his rescuer, then did thumbs-up with both arms while we were trying to load him to the gurney," Opliger said.

The worker remained in the hospital late Wednesday. It was not immediately known when he would be well enough to leave.

One dead after small planes collide mid-air in Calif.

    NBCLosAngeles.com

A single-engine Cessna lands near the third hole on Westlake Golf Course on Monday, April 29, 2013.

By Samantha Tata, Rosa Ordaz and Beverly White, NBCLosAngeles.com

A pilot died when a small plane collided mid-air with another plane that made an emergency landing on a nearby golf course in Southern California, according to the Federal Aviation Administration.

The planes, both Cessnas, were flying at altitudes above 3,000 feet when they collided about 8 miles east, northeast of Ventura, according to FAA radar data.

The first airplane was headed west at 3,500 feet. The second airplane was headed east at 3,100 feet. That plane had just departed Santa Monica for an engine test flight, said Allen Kenitzer, a spokesman with the FAA.

The first plane made an emergency landing after 2 p.m. near the third hole at the Westlake Golf Course in Westlake Village (map). The second airplane crashed into mountainous terrain in Calabasas, sparking a 1 acre brush fire.

The pilot in that crash died.

All three people aboard the plane that landed on the golf course survived, said Deputy Mark Pope, of the Los Angeles County Sheriff's Department. At least one of them was injured.

The plane is registered to AmeriFlyers, a Dallas-based flight school with a Santa Monica location.

The golf course was open at the time the plane made its rough landing, but no one on the ground was injured.

The sound of a low-flying aircraft scared golfers off the driving range and into the shop.

The second plane was found about the same time Monday as firefighters responded to a brush fire sparked by aircraft debris, about 5 miles away from the golf course.

U-Haul driver surrenders after high-speed chase through 2 California counties

By Samantha Tata and Alex Calder, NBCLosAngeles.com

The driver of a U-Haul truck led police on a high-speed chase over rain-soaked freeways and surface streets in two Southern California counties Wednesday night.

The pursuit began in Riverside and ended in Orange County, police said. The driver is believed to have tripped silent alarms at a U-Haul facility in Riverside during a robbery attempt earlier in the evening.

    Follow @NBCNewsUS

Driven by a man with no shirt, the U-Haul truck was traveling upwards of 90 mph on the westbound 91 Freeway through Anaheim Hills at 10:30 p.m. (1:30 a.m. ET) before transitioning onto the southbound 55 Freeway into Orange County.

    More news from NBCLosAngeles.com

About 15 minutes later, he changed directions and headed north on the 5 Freeway in the Santa Ana area before transitioning back onto the 91 Freeway, this time heading east.

Roads in the area were wet as the truck barreled down nearly empty freeways. The driver fishtailed on at least three slippery on-ramps.

It appeared California Highway Patrol officials successfully used a maneuver against the moving truck, causing it spin out and crash on a freeway underpass shortly after 11 p.m. (2 a.m. ET) along State College Boulevard on the border between Anaheim and Santa Ana.

Women mistakenly wounded in California manhunt to get $4.2M in settlement

    By Olsen Ebright, NBCLosAngeles.com

The city of Los Angeles will pay $4.2 million to the two women injured when police mistakenly opened fire on their truck during the February manhunt for a disgruntled ex-LAPD police officer who killed four people in a rampage that paralyzed Southern California.

LA City Attorney Carmen Trutanich confirmed Tuesday to NBC4 that a deal has been reached with Margie Carranza and her mother, Emma Hernandez.

"The deal is relatively a very simple, very clean deal. It's a win-win for both parties," Trutanich said. "It closes this chapter in Los Angeles and LAPD history on all issues."

The $4.2 million will be split between the two women "any way they want," Trutanich said.

The shooting occurred Feb. 7 during a manhunt for Christopher Dorner. Earlier in the day, two Riverside, Calif., officers were ambushed in their police car, and authorities were on the hunt for Dorner and his Nissan Titan pickup truck.

At about 5 a.m., as the women were delivering newspapers in their Toyota Tacoma pickup in city of Torrance, police encountered them and opened fire.

Hernandez, 71, was shot twice in the back, and Carranza, 47, was injured by broken glass.

The search for Dorner ended Feb. 12 with a shootout and standoff in the Big Bear area.

Dorner was holed up in a cabin surrounded by authorities when a police tear gas canister shot into the residence started a fire. Dorner then shot and killed himself, according to deputies.

Disneyland contractor fined $60,995 after worker hurt on Space Mountain

Matthew Simmons / Getty Images file

Crowds gather for the re-launch ceremony of Disneyland's Space Mountain attraction on July 15, 2005 in Anaheim, California.

California safety regulators have fined a Disney contractor $60,995 after a worker was injured while cleaning the Space Mountain ride.

The California Division of Occupational Safety and Health says Los Angeles-based HSG Inc. violated several safety rules. The fine was levied on Friday.

A 37-year-old contractor fell and suffered broken bones last November while cleaning the exterior of Space Mountain at Disneyland.

The Orange County Register reports the contractor was cited for failing to make sure anchors were provided to the worker.

HSG has two weeks to file an appeal. An email to the company was not immediately returned.

    More news from NBCLosAngeles.com

Earlier this week, Cal-OSHA proposed a nearly $235,000 penalty against Disneyland in connection with the worker's injury.

Disney has not decided whether it will appeal the penalties.

Man killed in fire attack spurned panhandler, witness says

    By Olsen Ebright, NBCLosAngeles.com

A 63-year-old man burned in his car outside a Long Beach 7-Eleven, possibly as retribution for spurning a homeless man, has died.

The victim, who was identified as Jerry Payne of Long Beach, died about 2 a.m. Monday at the hospital, according to the Long Beach Police Department.

See photo, original report at NBCLosAngeles.com

Raymond Sean Clark, a 38-year-old homeless man, was arrested on suspicion of throwing a flammable substance into the victim's car outside the convenience store.

"The suspect … will be re-booked for murder and a new bail has yet to be determined. Detectives still anticipate presenting the case to the District Attorney's Office within this week," according to a statement from police.

The attack happened Friday about 5:15 p.m. at the 7-Eleven store located near the intersection of Clark Avenue and Pacific Coast Highway.

Detectives have not established a motive in the alleged crime, but a witness told NBC4 the victim refused to give money to the panhandler. A store owner said the man has been seen in the area before and reported for loitering.
